Researchers Finally Put Pickleball to the Test

Pickleball players do not need a scientific study to tell them that the game gets them moving. Spend a morning on the court and that part is obvious. What has been much harder to answer is whether regular pickleball can produce measurable changes in the kinds of physical abilities that matter as people get older.

A new study took that question much further. “Effects of Pickleball on Pre-frailty, Physical Fitness, 24-Hour Movement Behaviors, and Quality of Life in Older Adults: A Randomized Controlled Trial,” published in the International Journal of Nursing Studies, followed older adults with pre-frailty through an eight-week pickleball program. The researchers measured not only whether their frailty status changed, but also strength, endurance, flexibility, daily movement and quality of life.

The results were striking. After eight weeks, 42 percent of the people assigned to pickleball were no longer classified as pre-frail, compared with 8 percent of the control group. The pickleball group also improved on several measures of functional fitness, spent less time sedentary and reported improvements in both physical and mental aspects of quality of life.

How the Study Was Done

This was a randomized controlled trial conducted at The Chinese University of Hong Kong. That matters because the researchers did more than recruit pickleball players and compare them with people who did not play. They enrolled 72 community-dwelling older adults who met the study’s criteria for pre-frailty and randomly assigned 36 to the pickleball program and 36 to a control group that continued its usual lifestyle.

The median age of the participants was 67, and about 60 percent were women. The pickleball group was scheduled for two supervised 60-minute sessions each week for eight weeks, for a total of 16 sessions. Certified instructors led the sessions, the player-to-instructor ratio was kept at eight to one or lower, and the program was designed to keep the activity at a moderate effort. Participants attended an average of 13 of the 16 sessions.

The researchers assessed pre-frailty using the Fried Frailty Phenotype, a commonly used framework that looks at five areas associated with frailty. They also used functional fitness tests, including how many times participants could rise from a chair in 30 seconds, arm curls, shoulder flexibility and how far they could walk in six minutes. Accelerometers were used to measure movement across the day, including sedentary time, light activity, moderate-to-vigorous activity and sleep. Quality of life was measured as well.

Because participants were randomly assigned to the two groups and the outcome assessors and data analysts were blinded to group assignment, the design gives the findings more weight than a survey asking existing players how pickleball makes them feel. The participants and instructors obviously knew whether they were playing pickleball, but the researchers took steps to reduce bias where the nature of the intervention allowed it.

The Finding That Stands Out

The headline result was the change in pre-frailty. Everyone entered the study classified as pre-frail. Eight weeks later, 42 percent of the pickleball group had moved into the study’s non-frail, or robust, category. Only 8 percent of the control group did the same. The researchers calculated an absolute recovery increase of 33 percent between the groups.

Pre-frailty may be an unfamiliar term to many players. It describes an intermediate state between being robust and being frail. It does not mean someone is unable to live independently or participate in normal activities. The importance is that it can signal reduced physical reserves and an increased vulnerability to future decline. It is also considered a stage where improvement may still be possible, which is exactly why the researchers chose to study it.

There is an important detail in the results. When the researchers examined the individual components used to determine frailty status, the only component showing a statistically significant difference between the groups was low physical activity. That makes the overall reversal finding interesting, but it also means the study should not be read as proof that pickleball independently corrected every component associated with pre-frailty.

Pickleball Improved More Than Activity Levels

The changes were not limited to the frailty classification. The pickleball group improved on several practical fitness tests. Participants completed more chair stands in 30 seconds, performed more arm curls, improved on the back-scratch shoulder flexibility test and increased the distance they could cover during a six-minute walk.

Those tests may sound simple, but they measure abilities that matter well beyond the pickleball court. Rising repeatedly from a chair reflects lower-body strength and function. The six-minute walk provides information about walking endurance. The other tests capture upper-body strength and flexibility. These are everyday physical abilities that help people continue moving independently as they age.

The researchers also found changes outside the scheduled pickleball sessions. The pickleball group reduced sedentary time and increased light physical activity. That is an especially interesting finding because the intervention itself only required two hours of pickleball per week. The study cannot tell us exactly why participants became less sedentary during the rest of their day, but the change suggests that the effect of joining an activity may extend beyond the time spent actually doing it.

Quality-of-life scores improved in both the physical and mental health areas as well. The study was not designed to determine precisely which parts of pickleball produced those changes. The exercise, learning a new skill, playing with other people and participating in a structured group activity may all have contributed.

What Players Should Take From the Study

For Fit Pickler readers who already play regularly, the takeaway is not that two pickleball sessions a week are a magic prescription. These participants were specifically selected because they had pre-frailty, and they took part in a supervised program with instructors, planned sessions and a moderate target intensity. The results cannot automatically be applied to every older adult or every kind of open play.

The study does show that pickleball can be substantial physical activity. The movements players make during a game require lower-body strength, endurance, coordination, balance and repeated changes of direction. When those demands are repeated consistently, the body has an opportunity to adapt.

It also reinforces the value of consistency. The program lasted only eight weeks, but participants were scheduled to play twice every week. For someone who wants the health benefits of pickleball, regular participation is likely more useful than an occasional marathon session followed by long periods of inactivity.

There is also no reason to conclude that pickleball should replace other forms of exercise. Strength training, balance and fall prevention work, walking and other activities can address physical needs that a pickleball game may not cover completely. The more useful lesson is that pickleball can be a meaningful part of an active lifestyle rather than simply something people do for fun.

The Study Had Limits

This was a relatively small study with 72 participants, all of whom were community-dwelling older adults with pre-frailty. The intervention lasted eight weeks, so it cannot tell us whether the improvements would continue over many months or years, or whether participants would maintain them if they stopped playing.

The control group continued its usual lifestyle rather than participating in another organized exercise or social program. That means the study shows how the pickleball program compared with usual life, but it does not tell us whether pickleball would outperform walking, strength training, another racquet sport or a different group exercise program.

There were no severe adverse events during the intervention. Three participants reported minor issues, including a shoulder strain, wrist pain and an episode of hypoglycemia, and all continued in the program after appropriate management. That is reassuring, but a study this size is too small to establish the overall injury risk of pickleball for older adults.

Final Thoughts

The most interesting part of this research is not simply that people exercised and became fitter. It is that a structured pickleball program produced measurable changes across several areas that matter with age, including functional fitness, daily movement and quality of life, while a substantial portion of the pickleball group moved out of the study’s pre-frail classification.

That does not make pickleball a treatment for aging, and it does not mean every player will experience the same results. It does give the sport something it has not always had: evidence from a randomized controlled trial showing that regular participation can produce meaningful changes in older adults.

For people who already love the game, that may be the best news in the study. The hours spent chasing dinks, moving through the transition zone and getting back for one more ball are not just time on the court. They can be part of the work that helps keep the body moving well away from it, too.
